Service Coordination UnitThe Service Coordination Unit is responsible for helping consumers and their families access services and ensuring that services are provided in an appropriate, timely, and effective manner. This is accomplished through assessment, service planning, quality assurance, utilization management, and case monitoring. Services include Clinical Intake, Administrative Service Coordination, Blended Service Coordination, and 24 Hour Emergency Services.

Service Coordination Unit
The Service Coordination Unit is responsible for helping consumers and their families access services and ensuring that services are provided in an appropriate, timely, and effective manner. This is accomplished through assessment, service planning, quality assurance, utilization management, and case monitoring.
Services Provided:

Clinical Intake – Consumers seeking Outpatient services are referred to the Intake Department. An Intake Clinician will complete an assessment, determine if our Out Patient Services may be able to appropriately address the needs presented, and schedule the initial appointment. If the caller is in need of services that are not provided by the Agency, the Intake Clinician will link the caller to appropriate resources.

Service Coordination (Case Management) – Services are available to all consumers residing in Allegheny County.

Administrative Service Coordination – Administrative Service Coordinators provide community outreach, linkage, service referral and advocacy. Administrative Service Coordinators assist both adult and child consumers in obtaining basic needs, provide linkage to needed resources and services, assist in securing benefits, and on-going monitoring and support.

Blended Service Coordination – Blended Service Coordinators provide an array of medically necessary, strength-based, case management services to eligible adults, children, and families. Service Coordinators provide 24-hour coverage to all consumers active in the program.

Emergency Services – Milestone offers an emergency on-call service that responds by telephone 24 hours a day, 7 days a week. This service includes immediate assessment and support for a consumer in crisis.
